This is an interesting paper reporting on an exploration of the differences in implementation of ART scaling up.  They think it is “useful to consider health systems as complex systems”, where simple rules can be used (on condition that the agents have the capacity, means, margins of freedom and the motivation to do so!). “The most effective course of action emerges with time through experimentation and learning during which innovations are adapted locally in an active process of achieving an ‘intervention-system’ fit”.
